pass trough and rename unused variables. needed to process classes compiled by coldfusion with invalid debug info

removed a TODO (type variables do not induce dependencies by themselves; type variable bounds can, but they are already managed)

removed TODOs and unused code (code for arrays was unused because annotation arrays are always decomposed with visitArray, and code for primitive types such as B, C, S was unused because annotation values of these types are represented with boxed types - and all this code is also unused for visitField and visitLdcInsn, the callers of getValue method)
